High AST Normal ALT: Understanding the Macro-AST Possibility
AST is present in several tissues, including the liver and skeletal muscle. An elevated result can therefore have more than one source. Macro-AST offers a different explanation: the enzyme binds to another molecule, usually an antibody, creating a larger complex that is cleared from the bloodstream more slowly. The resulting increase in measured AST activity may persist without ongoing tissue injury.
The discussion focuses on the clues that make macro-AST worth considering, especially repeated AST elevation alongside otherwise reassuring findings. Normal ALT alone does not establish the diagnosis or exclude liver disease. Medical history, symptoms, medications, recent exercise, and other laboratory results still matter.
What Can Specialized Testing Reveal?
Learn how polyethylene glycol, or PEG, precipitation helps laboratories investigate macro-AST and why more specialized methods, such as gel-filtration chromatography, may be useful. Refrigerating a sample at approximately 4°C can lower AST activity in some cases, but other macro-AST complexes remain stable. A negative refrigeration test therefore does not rule out the condition.
